- Mild nondisabling stroke (NIHSS score 0–5)
- Acute intracranial hemorrhage
- Ischemic stroke within 3 months
- Severe head trauma within 3 months
- Acute head trauma
- Intracranial/intraspinal surgery within 3 months
- History of intracranial hemorrhage
- GI malignancy or GI bleed within 21 days
- Coagulopathy
- On anticoagulants (LMWH or DOACs)
- Infective endocarditis
- Aortic arch dissection
- Intra-axial intracranial neoplasm
